1. Information We Collect
A. Name, Address, and
Phone
When you place an order, we require the order recipient's
name and shipping address, and optionally, telephone number. We also require the
credit card holder's name, billing address and phone number. We use this
information solely to process and fulfill your order.
B. Email Address
We require your email address when you place an order, ask
us a question via email, or subscribe to our newsletter.
We use your email address to contact you about the status
of your order, reply to your question, or to send you periodic announcements if
you have subscribed to our newsletter It is not used for any other purpose and
it is not shared with any third parties.
When you subscribe to our newsletter, you must confirm
your subscription by following the instructions in a confirmation email that is
sent to you, or you won't receive any alerts. Once confirmed, you can
unsubscribe at any time by modifying your account profile
C. IP Address
An IP address is a unique number assigned to either your
computer, your broadband router, your business network or your Internet service
provider. Your IP address may reveal information about you, such as where you
work. We collect your IP address when you place an order and in our web logs for
each page you visit. We use your IP address solely to help verify the
authenticity of an order.
D. User Name
You are assigned a unique id, called a user name, when you
first enter our store. We store this in your order and in our web logs for each
page you visit. We use your user name to keep track of your shopping cart.
E. Affiliate Code and
Incentive ID
If you clicked a link on another site to enter Fifth
Avenue Lingerie, that site may have passed a Affiliate Code to us that allows us
to identify the site and/or link and/or search engine keyword you typed. In
addition, if you have registered at the referring site to receive rebates or
other incentives, the referring site may pass us a unique ID that they assigned
to you, which we will call the Incentive ID. The Affiliate Code and Incentive ID
are stored in your order and in our web logs for each page you visit. We use
this information to pay commissions to referring sites and to determine the
profitability of our online advertising campaigns.
F. Web Log
Our Internet server records in a log the following
information for each page you visit at our site: the URL, your IP address, the
referring site URL and all cookies created by our site. We use this information
only to determine who is providing us the most visitors to our website (as in,
total visitors to our site yesterday, total visitors that found our store
through Google, etc.) and we delete our log files within 72 hours. This
information is not shared with any third parties.
2. Third Party Sharing
Other than the sharing of information described in this
section, we do not share your personally identifiable information with any other
third parties or for any reasons not described, except: We reserve the right to
disclose your personally identifiable information as required by law and when we
believe that disclosure is necessary to protect our rights and/or comply with a
judicial proceeding, court order, or legal process served on our Web site.
A. Credit Card Processor
We share your credit card billing address and credit card
information with our credit card provider for the purpose of charging your
credit card for any order you place. We may also share your IP address to
investigate fraudulent charges. Our credit card provider is not authorized to
use this information for any other purpose.
B. Shipping Carrier
We share the recipient's name (the person you ship an
order to), shipping address and phone number (if provided) with the shipping
carrier so they can deliver the package. The shipping carriers are not
authorized to use this information for any other purpose.
3. Techniques We Use to
Collect/Share Information
In addition to collecting information via forms, such as
the ones you fill out during the online checkout process, here are the other
methods we use to collect information that you may not be aware of.
A. Cookies
Cookies are small files of data, sometimes containing
personally identifiable information, that are stored on your computer by most
web sites that you visit. We use cookies to store your user name, Affiliate
Code, Incentive ID, and your email address if you choose to have our store
remember it for ease in filling out forms. If you disable cookies in your
browser, you can still browse our store and place an order, but some features
will be disabled.
B. URL Parameters
We collect the Affiliate Code and Incentive ID by reading
parameters embedded in the URL of the link you clicked to enter our store. We
also pass your user name from page to page within our store, which is the only
way you would be able to place an order if your cookies were disabled.
C. Clear GIFs
A clear gif is a small, usually one pixel, graphic that is
invisibly embedded on a web page. We use this method on the order receipt page
to pass the Incentive ID, order number and order total to our affiliate
providers.
4. Security
We utilize industry standard SSL encryption when accepting
your name, address and credit card information during our online checkout
process. In addition, we utilize the same encryption standard when transferring
your credit card data to our internal fulfillment system. To further enhance our
security measures, our fulfillment system is not accessible via the Internet and
we delete from our web site database all credit card information within 24 hours
of receipt.
We have also taken the appropriate security measures to
safeguard your personal information after it has been transferred to our
fulfillment system. For example, our fulfillment system is password protected,
it is inaccessible via the Internet, data backups are encrypted and paper
copies, if any, are shredded before disposal.
5. Changes/Deletions

6. Notification of
Changes to our Privacy Policy
If we are going to use your personally identifiable
information in a manner different from that stated at the time of collection we
will notify you via email. You will have a choice as to whether or not we use
your information in this different manner. In addition, if we make any material
changes in our privacy practices that do not affect user information already
stored in our database, we will post a prominent notice on our web site
notifying users of the change. In some cases where we post the notice we will
also email users, who have opted to receive communications from us, notifying
them of the changes in our privacy practices.
